Immigration Medical Exam - Plano, Texas

Dr. Isaac Z. Pugach, M.D. is fully approved by USCIS to perform immigration medical exams, and he is considered a Designated Civil Surgeon, or immigration medical specialist, by United States Citizenship and Immigration Services.

Immigration physical exam is required by USCIS (U.S. Citizenship and Immigration Services) for certain individuals seeking permanent residence in the United States.  This examination must be done by a medical doctor who is approved by USCIS to perform such physicals.  After the performance of the immigration physical, if all the requirements are met, an I-693 form will be completed.

This exam requires applicants to be free of certain infectious diseases, including tuberculosis (TB), syphilis, and gonorrhea which will require blood and urine testing.  Although HIV was a mandatory test in the past, the law has changed, and since 2010, HIV is no longer required for immigration physical.  In addition, applicants must have received age appropriate vaccinations, as recommended by CDC (Centers for Disease Control).

If the applicant is found to have an infectious condition that is of a public health concern, it must be evaluated (eg chest X-ray, additional labs) and treated before applicant can proceed to the next step in the immigration process.

Common medical conditions that are not a public health concern, such as diabetes or high blood pressure, are acceptable for USCIS; therefore, having these conditions will not interfere with the immigration process, and the civil surgeon will complete the i693 form.

What to bring and what to expect:


• We will give you a current I-693 form to fill out at the office, or you can complete and print out your form and bring it to the office (do not sign it).

• Bring a government picture ID, such as a driver's license, ID card, or a passport.

• Bring records of vaccinations with you, if you have them. If you do not have records, it is not a problem - we will be able to give the vaccinations in the office. If you have health insurance, most likely you can get the vaccinations free of charge. We can also test for immunity, if you had vaccines before.

• Examination cost includes physical exam, testing for syphilis, TB, gonorrhea, and sealed I-693 form, as required by USCIS. Exam is not covered by insurance, but the vaccines usually are.
